Behavioral Health
Tri State Case Management Program serves as a means for achieving client wellness autonomy through advocacy, innovation, communication, education, identification of service resources and service facilitation.
What is Integrated Behavioral Health?
Primary care settings have become a gateway for many individuals with behavioral health and primary care needs. To address these needs, many primary care providers are including behavioral health care services into their setting. Combining mental health, substance abuse, and primary care services produces the best outcomes and proves the most effective approach to caring for people with multiple healthcare needs.
